We are located in southeast Idaho in the town of Pocatello, approximately 150 miles north of Salt Lake or 3.5 hours east of Boise. 

Pocatello Creek Clinic - Exit 71 and turn West. 
Take your first left and left again into the office park.  We are the second building in the office park on the ground floor.
